Background:

Intravitreal injections are a standard treatment for wet age-related macular degeneration.
Complications, though rare, require careful consideration and management.

Observation:

An 84-year-old woman developed a Seidel-positive conjunctival cyst after multiple intravitreal injections.
The cyst was suspected to be an inadvertent filtering bleb caused by a full-thickness scleral perforation.

Findings:

Surgical closure of the fistula and bleb repair were successfully performed.
Histopathological examination confirmed the nature of the lesion.

Implications:

Highlights the importance of recognizing inadvertent filtering blebs as a potential complication of intravitreal injections.
Emphasizes the need for vigilant monitoring and timely surgical management to prevent vision impairment.
