Summary

Objective manikin assessments of cardiopulmonary resuscitation (CPR) quality are more reliable than subjective peer assessments. This study found observers consistently overestimated chest compression quality compared to manikin data, supporting the use of objective feedback in CPR training.

Purpose of the Study:

  • To compare subjective, peer-led chest compression assessments with objective manikin data.
  • To evaluate assessment variability across different instructor backgrounds.

Main Methods:

  • A quantitative, multi-center study involving 78 instructors in simulated CPR scenarios.
  • Participants performed 2-minute chest compressions, assessed by peers and a Brayden PRO® manikin.
  • Data collected from 13,227 chest compressions.

Main Results:

  • Manikin median score: 88.5%; observer median score: 92%.
  • Poor correlation between subjective and objective assessment methods (Kappa -0.051 to +0.07).
  • Manikin scores showed good internal consistency (alpha=0.789) versus observer scores (alpha=0.011).

Conclusions:

  • Observers consistently rated chest compression quality higher than objective manikin assessments.
  • Findings support the increased use of objective assessment tools in CPR training.
  • Objective feedback is recommended regardless of instructor background for improved CPR quality.
