Bones of the Upper Limb: Humerus01:19

Bones of the Upper Limb: Humerus

5.9K
The upper limb consists of the arm, forearm, wrist, and hand bones. The humerus is the single bone of the upper arm region. Proximally, it has a large, spherical, smooth head that articulates with the glenoid cavity of the scapula to form the glenohumeral or shoulder joint. The margin of the head is the anatomical neck, a residual epiphyseal plate. Laterally it extends to form bony projections called the greater tubercle and the lesser tubercle. Next to the tubercles is the surgical neck, a...

Muscles that Move the Arm01:31

Muscles that Move the Arm

3.8K
Nine muscles are involved in arm movements. Two of these, the pectoralis major and latissimus dorsi, originate from the axial skeleton and are called axial muscles. The other seven originate from the scapula and are called the scapular muscles.
The pectoralis major has two origins. Its clavicular head originates on the medial half of the clavicle. In contrast, the sternocostal head originates on the costal cartilages of ribs 1-6, the sternum, and the aponeurosis of the external oblique of the...

Summary

The triceps-sparing approach offers excellent exposure for humerus fracture repair, minimizing risk to the radial nerve. This muscle-sparing technique allows effective fixation of humeral shaft and distal humerus fractures without damaging the triceps muscle.

Area of Science:

  • Orthopedic Surgery
  • Surgical Techniques
  • Trauma Management

Background:

  • Posterior humerus fractures require surgical intervention for optimal outcomes.
  • Traditional approaches may pose risks to surrounding neurovascular structures.
  • The triceps muscle is a key anatomical consideration in posterior humerus surgery.

Observation:

  • A novel triceps-sparing surgical approach was utilized.
  • This technique involves a muscle-splitting interval to access the posterior humerus.
  • The approach was applied to a case of extra-articular distal humerus fracture.

Findings:

  • The triceps-sparing approach provides broad surgical exposure of the posterior humerus.
  • This method allows for effective reduction and fixation of humeral shaft fractures.
  • It also facilitates fixation of extra-articular distal humerus fractures.
  • The approach demonstrated a low risk of radial nerve palsy.

Implications:

  • This muscle-sparing technique offers a safe and effective alternative for humerus fracture fixation.
  • It may reduce the incidence of iatrogenic radial nerve injury.
  • The approach is beneficial for treating both humeral shaft and distal humerus fractures.
  • Video documentation aids in surgical education and technique dissemination.
