Summary

High oxyphil cell (OxC) content in parathyroid glands doesn't affect calcium levels in primary hyperparathyroidism (PHPT). However, it may indicate pathologic glands and worsen kidney function.

Background:

  • The role of oxyphil cells (OxC) in primary hyperparathyroidism (PHPT) is debated, with historical views considering them involuted but potential roles in hormone secretion suggested.
  • Clinical characteristics of OxC-rich adenomas and localization tests in PHPT require further investigation.

Purpose of the Study:

  • To analyze the implications of oxyphil cell content in parathyroid glands of patients with primary hyperparathyroidism.

Main Methods:

  • A retrospective cohort study included 238 patients undergoing parathyroidectomy for PHPT and normal glands.
  • Researchers performed semi-quantitative analysis of oxyphil cell percentage, comparing groups with <25% OxC and >75% OxC.

Main Results:

  • Patients with >75% OxC parathyroid adenomas (OxCA) showed worse preoperative glomerular filtration rate (p=0.043).
  • Ultrasound accuracy for localization was lower in the high OxC group (48.3% vs. 73.7%; p=0.035).

Conclusions:

  • High oxyphil cell content in parathyroid glands does not influence phosphocalcic metabolism in PHPT.
  • Elevated OxC content appears exclusive to pathologic parathyroid glands and may correlate with renal function decline in PHPT patients.
