The modified Stoppa approach, with or without the iliac wing approach, offers better outcomes for acetabular fractures in polytrauma patients. This technique may reduce the risk of post-traumatic osteoarthritis and the need for hip replacement.
Area of Science:
- Orthopedic Surgery
- Trauma Surgery
Background:
- Acetabular fractures in polytrauma patients are associated with high-energy impact.
- These fractures can lead to severe post-traumatic osteoarthritis (PTOA) and necessitate hip replacement.
Purpose of the Study:
- To evaluate the outcomes of polytrauma patients with acetabular fractures treated with the modified Stoppa approach, alone or combined with the iliac wing approach.
- To compare these outcomes with those achieved using the ilioinguinal approach.
Main Methods:
- Retrospective study of 42 polytrauma patients with acetabular fractures treated between 2008-2018.
- Fracture reduction and outcomes assessed using the Majeed score.
- Analysis of hip replacements due to PTOA.
Main Results:
- 92% of acetabular fractures achieved anatomical or satisfactory healing.
- The modified Stoppa group showed significantly better Majeed scores (p<0.05).
- Seven patients required hip replacement due to PTOA; outcomes varied based on reduction quality.
Conclusions:
- The modified Stoppa approach and its combination with the iliac wing approach yield good radiological and functional results.
- These techniques may decrease the risk of severe PTOA and hip replacement in polytrauma patients.
- Unsatisfactory fracture reduction is a strong predictor of symptomatic PTOA requiring total hip arthroplasty.
