Local Anesthetics: Clinical Application as Intravenous Regional Anesthesia

723
Intravenous regional anesthesia or the Bier block technique is used to anesthetize a specific limb or extremity. It uses exsanguinated or blood-drained vessels to transport local anesthetics or LAs to the peripheral nerve trunks. Lidocaine without vasoconstrictors like epinephrine is most commonly used for this technique. Other drugs used are prilocaine, ropivacaine, and chloroprocaine. Bupivacaine is not recommended for this technique due to its high cardiac toxicity.

Area of Science:

  • Oncology
  • Anesthesiology
  • Surgical Innovation

Background:

  • Melanoma axillary sentinel node biopsy (ASNB) typically requires general anesthesia (GA).
  • GA is associated with increased mortality risks during the COVID-19 pandemic.
  • Regional blocks offer a safer alternative for ASNB, avoiding GA risks.

Discussion:

  • A prospective study assessed the feasibility of ASNB under regional blocks in ten melanoma patients.
  • Patients reported high satisfaction and no significant difference in Quality of Recovery-15 scores.
  • A significant reduction in postoperative hospital stay was observed.

Key Insights:

  • Regional blocks are a safe and feasible alternative to GA for ASNB.
  • This approach mitigates GA-related risks during the COVID-19 pandemic.
  • ASNB under regional block leads to shorter hospital stays and improved patient satisfaction.

Outlook:

  • Further research may explore wider adoption of regional blocks for ASNB.
  • Optimizing patient selection and surgical technique is crucial for successful implementation.
  • This practice change highlights successful collaboration between anesthesia and surgical teams.
