Combinatorial treatment for unresectable unicentric Castleman disease

Background:

• Unresectable, symptomatic unicentric Castleman disease (UCD) presents significant treatment challenges.
• Highly vascularized UCD masses present an opportunity for therapeutic embolization.

Observation:

• This study reports on 6 patients with unresectable UCD treated with therapeutic embolization combined with other interventions.
• Interventions included surgery, rituximab, cryoablation, and chemotherapy.

Findings:

• Five patients achieved significant tumor volume reduction (median 83.2%).
• All responding patients experienced resolution of symptoms.
• No serious complications were observed in patients undergoing embolization followed by surgery.

Implications:

• Multimodal therapy, including embolization, offers effective disease and symptom control for unresectable UCD.
• This approach provides a viable strategy for managing challenging UCD cases.
