Air leaks, including pneumothorax, can occur in coronavirus disease 2019 (COVID-19) patients even without mechanical ventilation. Mortality risk significantly increases when air leaks develop alongside positive pressure ventilation or require advanced respiratory support.

- Mortality was higher in patients with air leaks associated with positive pressure ventilation (49%) and escalated respiratory support (39%).
Conclusions:
- Air leaks in COVID-19 can manifest spontaneously, independent of positive pressure ventilation or pre-existing risk factors.
- Positive pressure ventilation and the need for escalated respiratory support significantly increase mortality risk in COVID-19 patients with air leaks.
