Cyclin-dependent kinase 4/6 (CDK 4/6) inhibitors are effective for metastatic breast cancer but can cause lung injury. This case highlights the need for awareness and understanding of managing this rare but serious side effect.

  • A case of a female patient with metastatic hormone receptor-positive/human epidermal growth factor 2-negative breast cancer is presented.
  • The patient developed lung toxicity during treatment with ribociclib, a CDK 4/6 inhibitor.

  • Lung injury is an identified, albeit less common, toxicity associated with CDK 4/6 inhibitors.
