Background:

  • Odontogenic sinusitis (ODS) poses risks of severe orbital, intracranial, and osseous complications.
  • Current sinusitis guidelines lack detailed discussion on diagnosing and managing complicated ODS.

Purpose of the Study:

  • To systematically review epidemiological and clinical features of complicated ODS.
  • To describe management strategies for complicated ODS.

Main Methods:

  • Systematic review of studies from PubMed, EMBASE, and Cochrane Library.
  • Qualitative analysis of all complicated ODS studies; quantitative analysis for studies with patient-level data on orbital and intracranial complications.

Main Results:

  • 75 studies (110 cases) included; 70% involved orbital complications.
  • Complications primarily arose from apical periodontitis of maxillary molars; anaerobic and streptococcal bacteria were common.
  • Management involved antibiotics and surgery for complications and infectious sources.

Conclusions:

  • ODS must be considered in patients with infectious extrasinus complications.
  • Multidisciplinary collaboration among otolaryngologists, dentists, ophthalmologists, and neurosurgeons is recommended for optimal patient outcomes.
