Case Report: Systemic Sclerosis After Covid-19 Infection.
1Department of Public Health and Caring Sciences, Unit of General Practice, Uppsala University, Uppsala, Sweden.
The coronavirus disease (COVID-19) can trigger autoimmune conditions. This case report details a patient who developed systemic sclerosis (SSc) after a mild SARS-CoV-2 infection, highlighting a potential new autoimmune complication.
Area of Science:
- Immunology
- Rheumatology
- Infectious Diseases
Background:
- Coronavirus disease (COVID-19), caused by SARS-CoV-2, is a respiratory illness with potential long-term complications.
- Emerging evidence suggests COVID-19 can trigger autoimmune responses, including autoantibody production.
- Autoimmune diseases may manifest or worsen in genetically susceptible individuals post-COVID-19 infection.
Observation:
- A 47-year-old man experienced mild COVID-19 symptoms, testing positive for SARS-CoV-2.
- Three weeks post-infection, he developed symptoms suggestive of systemic sclerosis (SSc), including skin manifestations, Raynaud's phenomenon, and gastrointestinal issues.
- Laboratory tests revealed positive antinuclear antibodies (ANA) with a nucleolar pattern and positive anti-PM/Scl antibodies. High-resolution computed tomography (HCRT) indicated early interstitial lung disease (ILD).
Findings:
- The patient met the ACR/EULAR classification criteria for SSc, with a total score of 10.
- This case represents the first documented instance of new-onset systemic sclerosis following a COVID-19 infection.
- The findings suggest a potential causal link between SARS-CoV-2 infection and the development of SSc.
Implications:
- SARS-CoV-2 infection may be a trigger for systemic sclerosis in susceptible individuals.
- This expands the spectrum of known autoimmune complications associated with COVID-19.
- Further research is warranted to understand the mechanisms underlying COVID-19-induced SSc and its long-term prognosis.
