Insulin: Biosynthesis, Chemistry, and Preparation

707
The endoplasmic reticulum (ER) of pancreatic β-cells synthesizes preproinsulin, which consists of a signal peptide, A and B chains, and a C-peptide. Preproinsulin is then cleaved and folded into proinsulin, which translocates to the Golgi apparatus for sorting and packaging into secretory granules. In these granules, enzymatic clipping generates insulin and C-peptide.
Damage or functional impairment of β-cells inhibits insulin production, leading to diabetes. Diabetes treatment...

    Summary
    This summary is machine-generated.

Patients are increasingly using do-it-yourself artificial pancreas systems (DIY-APS) for diabetes management. While generally positive, these unapproved systems pose challenges for endocrinologists navigating patient safety and regulatory concerns.

    Area of Science:

    • Endocrinology
    • Biomedical Engineering
    • Patient-Centered Technology

    Background:

    • Growing patient adoption of do-it-yourself artificial pancreas systems (DIY-APS) for diabetes management.
    • DIY-APS integrate commercial continuous glucose monitors and insulin pumps with patient-developed algorithms.
    • These systems bypass formal safety studies and U.S. Food and Drug Administration (FDA) approval.

    Purpose of the Study:

    • To describe the current landscape of DIY-APS technology.
    • To outline regulatory considerations surrounding DIY-APS.
    • To provide recommendations for endocrinology providers regarding DIY-APS use.

    Main Methods:

    • Review of existing DIY-APS platforms (Loop, OpenAPS, AndroidAPS).
    • Analysis of patient-reported outcomes and experiences.
    • Examination of FDA communications and regulatory status.

    Main Results:

    • Worldwide patient adoption of DIY-APS is significant, often preferred over commercial options.
    • Patients generally report positive outcomes with DIY-APS.
    • The FDA issued a public warning regarding the use of these systems in May 2019.

    Conclusions:

    • Endocrinology providers face a complex decision regarding support for patients using DIY-APS.
    • Balancing patient autonomy with safety and regulatory guidelines is crucial.
    • Further guidance is needed for healthcare professionals managing patients on DIY-APS.
