Texture analysis of T2 maps can differentiate cartilage repair types. Matrix-associated chondrocyte transplantation repair tissue showed more mature collagen organization than microfracture repair tissue two years post-surgery.
Area of Science:
- Orthopedics and Sports Medicine
- Radiology and Medical Imaging
- Biomaterials Science
Background:
- Cartilage defects are common, impacting joint function and quality of life.
- Current cartilage repair procedures, such as microfracture (MFX) and matrix-associated chondrocyte transplantation (MACT), aim to restore hyaline cartilage.
- Assessing the maturation and quality of repair tissue is crucial for predicting long-term outcomes.
Purpose of the Study:
- To investigate the utility of texture features derived from T2-weighted magnetic resonance (MR) imaging maps as a quantitative marker for distinguishing the maturation of cartilage repair tissue.
- To compare the texture characteristics of repair tissue following MFX versus MACT procedures at two years post-surgery.
Main Methods:
- Seventy-nine patients undergoing either MFX or MACT for cartilage repair were evaluated using 3-T MR imaging, including T2 mapping, two years after surgery.
- Twenty-one texture features were extracted from the gray-level co-occurrence matrix (GLCM) of the T2 maps.
- Statistical analyses, including linear regression and intra-observer variability, were employed to assess differences and reproducibility of texture features between repair groups and anatomical locations (femoral condyle, trochlea/anterior condyle).
Main Results:
- No significant difference in T2 values was observed between MFX and MACT groups (P = 0.96).
- Significant differences in specific GLCM texture features were found between MFX and MACT repair tissues in the femoral condyle (autocorrelation, sum of squares, sum average, sum variance, sum entropy) and the anterior condyle/trochlea (homogeneity, dissimilarity).
- Texture analysis demonstrated higher sensitivity in differentiating repair tissue characteristics compared to T2 values alone.
Conclusions:
- Texture analysis of T2 maps, using GLCM, enhances the characterization of cartilage repair tissue by providing insights into structural organization.
- Certain texture features effectively distinguish between repair tissue resulting from MFX and MACT procedures.
- Repair tissue following MACT exhibited texture characteristics, indicative of collagen organization, that more closely resembled healthy cartilage compared to MFX repair tissue at 24 months post-intervention.
