Background:

  • Floating knee injuries (FKIs) are severe, high-velocity trauma involving both sides of the knee joint.
  • Outcomes are influenced by intra-articular fractures, comminution, soft tissue damage, and associated injuries.
  • Existing classifications do not fully capture all prognostic factors.

Purpose of the Study:

  • To analyze the efficacy of a novel conglomerated comprehensive classification system for FKIs.
  • To assess its ability to predict hospitalization duration, procedural needs, functional outcomes, and complications.

Main Methods:

  • A new classification system was developed, integrating existing standards (Fraser, AO, Gustilo-Anderson) with modifiers (M1-M3).
  • 92 FKIs were prospectively analyzed between 2015-2018 using the classification.
  • Functional outcomes were assessed using the modified Karlstrom-Olerud score post-union.

Main Results:

  • Significant differences were observed between classification groups I and IV regarding hospitalization duration (p=0.006) and number of procedures (p=0.018).

Conclusions:

  • Multicenter validation is recommended to further establish its utility.
