Area of Science:

  • Medical ethics
  • Health equity
  • Surgical care disparities

Background:

  • Incarcerated patients are a vulnerable population facing healthcare disparities.
  • Historical mistrust and security measures (shackles, guards) impede research and care.
  • Limited literature exists on the impact of these measures on healthcare workers and patient outcomes.

Purpose of the Study:

  • To assess surgical trainees' perceptions of health inequities and disparities in incarcerated individuals undergoing surgical care.
  • To identify challenges faced by healthcare providers in delivering care to incarcerated patients.

Main Methods:

  • Anonymous cross-sectional survey administered to general surgery trainees at a single institution.
  • Survey included 10 items: 9 yes/no and 1 open-ended question.
  • Data analyzed by averaging responses and reporting percentages.

Main Results:

  • 66% of trainees completed the survey; 97.5% have cared for incarcerated patients.
  • Many trainees reported operating with armed guards (62.5%) or caring for shackled, sedated patients (65%).
  • 75% witnessed law enforcement questioning during trauma evaluation; 47.5% believed incarcerated patients received substandard care.

Conclusions:

  • Healthcare for incarcerated patients requires urgent quality improvement.
  • Surgical trainees perceive significant challenges and substandard care for this population.
  • Further research is needed to understand and address these disparities in care delivery and follow-up.
