Summary

Acute renal failure in preeclampsia-eclampsia is often underestimated. Cortical necrosis, a severe kidney complication, was more frequent than expected, particularly when abruptio placentae co-occurred.

Background:

  • Acute renal failure (ARF) is considered uncommon and reversible in preeclampsia-eclampsia (PE-E).
  • Cortical necrosis, a severe form of ARF, is rarely reported in PE-E cases.
  • Mechanisms underlying ARF in PE-E remain poorly understood.

Purpose of the Study:

  • To investigate the incidence and clinical correlates of ARF, specifically cortical necrosis, in patients with PE-E.
  • To identify factors associated with severe renal impairment in PE-E.

Main Methods:

  • Retrospective analysis of 17 cases of PE-E with renal impairment over 15 years.
  • Clinical data review including patient demographics, pregnancy details, and co-existing conditions.
  • Histological and clinical diagnosis of cortical necrosis.

Conclusions:

  • Cortical necrosis is a more frequent complication of PE-E than previously thought.
  • The combination of PE-E and AP presents a particularly poor prognosis for renal function due to synergistic damage mechanisms.
  • Underdiagnosis of PE-E when AP is the initial symptom may lead to underestimation of severe renal damage.
