Background:

  • Munchausen syndrome is a complex factitious disorder characterized by intentional self-harm or illness simulation.
  • While behavioral aspects are well-documented, underlying neurological correlates are infrequently explored.

Observation:

  • Five patients diagnosed with Munchausen syndrome were assessed.
  • Despite appearing intellectually intact with strong verbal skills, formal neuropsychological evaluations were conducted.

Findings:

  • Neuropsychological testing identified significant deficits in conceptual organization.
  • Impairments were also noted in managing complex information and judgment.
  • These subtle cognitive impairments were present despite intact general intellectual functioning.

Implications:

  • Subtle neuropsychological dysfunction may be a significant contributing factor to the aberrant behaviors observed in Munchausen syndrome.
  • Behavibilities like irascibility, persistent help-seeking, and pseudologia fantastica could be manifestations of underlying brain damage.
  • Further research into the neurobiological underpinnings of Munchausen syndrome is warranted.
