Background:

Essential Tremor (ET) is a common movement disorder with ongoing debates regarding its definition and pathophysiology.
Recent criteria from the International Parkinson's and Movement Disorders Society (IPMDS) Tremor Task Force aim to clarify ET.
This review examines developments in ET understanding since the new classification's publication.

Purpose of the Study:

To analyze the impact of the new IPMDS criteria on the understanding of Essential Tremor.
To identify new developments in ET research since November 2017.

Main Methods:

A comprehensive Medline search was conducted on June 15, 2019, using the keyword "Essential Tremor".
English-written articles published after November 2017 were selected.
Relevant reviews, letters, and original studies were analyzed thematically: clinical characteristics, epidemiology, genetics, pathology, biomarkers, and treatment.

Main Results:

Out of 132 publications, 65 relevant articles were reviewed.
The majority of studies focused on surgical treatments, with fewer addressing epidemiology, pathology, or pathophysiology.
The new IPMDS classification is not yet widely implemented, except in recent therapeutic studies.

Conclusions:

The new Essential Tremor classification is not universally adopted, reflecting ongoing debate.
A key advancement is recognizing ET as a heterogeneous condition and introducing the "ET-plus" category.
Further sub-classification of ET-plus is proposed for improved diagnostic and therapeutic strategies.
