Area of Science:

  • Surgical innovation in abdominal wall reconstruction.

Background:

  • Diastasis recti is an anterior abdominal wall abnormality due to linea alba attenuation.
  • Severe diastasis recti involves attenuation of both linea alba and linea semilunaris.
  • Treatment options range from exercises to surgical repair, with or without mesh.

Purpose of the Study:

  • To review the indications and technique of onlay mesh for severe diastasis recti correction.
  • To evaluate the safety and efficacy of onlay mesh in abdominoplasty with diastasis repair.

Main Methods:

  • A retrospective review of 63 women undergoing abdominoplasty with diastasis repair (Jan 2010-Jan 2020).
  • Focus on 4 patients with severe diastasis treated with plication and onlay mesh (polypropylene or silk).
  • Indications for mesh included severe diastasis requiring anterior rectus sheath reinforcement.

Main Results:

  • All 4 patients tolerated the onlay mesh procedure well, with no reported morbidity.
  • Successful restoration of natural abdominal contour was achieved in all patients.
  • No instances of infection, seroma, delayed healing, or mesh removal were observed; drains removed within 7 days.

Conclusions:

  • Onlay mesh demonstrated success in surgical management of severe diastasis recti.
  • This technique should be considered for select patients requiring enhanced anterior rectus sheath strength.
