Summary

Adrenal leiomyomas are rare smooth muscle tumors that are difficult to distinguish from malignant adrenal masses. Prompt surgical resection is recommended due to non-specific imaging findings.

Background:

  • Adrenal leiomyomas are infrequent smooth muscle tumors.
  • They are challenging to differentiate from malignant adrenal tumors due to non-specific clinical and imaging findings.

Purpose of the Study:

  • To report a case of adrenal leiomyoma.
  • To highlight the diagnostic challenges and treatment of adrenal leiomyomas.

Main Methods:

  • A case of a 49-year-old male with abdominal pain and an adrenal mass is presented.
  • Diagnostic workup included ultrasonography and Contrast-enhanced Computerized Tomography (CECT).
  • Histopathological examination and immunohistochemistry confirmed the diagnosis of adrenal leiomyoma.

Main Results:

  • CECT revealed an uneven soft tissue density mass in the retroperitoneal region.
  • Histopathology showed a mesenchymal tumor, confirmed as leiomyoma by immunohistochemistry.
  • The tumor cells stained positive for desmin and smooth muscle actin.

Conclusions:

  • Adrenal leiomyomas present as heterogeneously enhancing masses on imaging.
  • Diagnosis is challenging due to non-specific imaging characteristics.
  • Prompt surgical resection is the primary treatment modality for adrenal masses.
