Background:

Internal herniation is a rare condition where abdominal organs protrude through peritoneal or mesenteric defects.
Defects in the broad ligament of the uterus are an uncommon site for internal herniation.

Observation:

Two cases of middle-aged women presenting with colicky lower abdominal pain were diagnosed with internal herniation through a broad ligament defect.
Clinical presentation included severe, intermittent abdominal pain suggestive of bowel obstruction.

Findings:

Internal herniation through the broad ligament is a rare etiology of abdominal pain.
The condition's origin remains unknown, with proposed congenital and acquired factors.

Implications:

Clinicians should consider broad ligament internal herniation in the differential diagnosis of unexplained abdominal pain in women.
Further research into the etiology and optimal management of this rare condition is warranted.
