In open-angle glaucoma, the iridocorneal angle remains open, but the trabecular meshwork becomes stiff, slowing down the outflow of aqueous humor. This causes a buildup of aqueous humor in the anterior chamber, leading to a sudden increase in intraocular pressure. Summary

Orbital cavernous hemangiomas are common tumors. The modified orbitozygomatic approach offers a safe and effective surgical option for large tumors, improving vision and cosmetic outcomes.

  • Transcranial and lateral orbitotomy approaches are traditional surgical methods for these tumors.

Observation:

  • A 62-year-old male presented with progressive proptosis and visual impairment due to a large superolateral intraconal cavernous hemangioma.
  • MRI confirmed the tumor compressing the optic nerve.

Findings:

  • A one-piece modified orbitozygomatic approach enabled complete tumor resection.
  • Postoperative outcomes included proptosis resolution, improved eye movement, and better visual acuity.

Implications:

  • The orbitozygomatic approach provides excellent surgical access and visualization for large orbital cavernous hemangiomas.
  • This technique facilitates safe tumor dissection and satisfactory orbital decompression.
