Lower blood B-cell lymphoma 2 (Bcl-2) concentrations in sepsis patients are linked to higher mortality. This study suggests Bcl-2 levels may help predict septic patient survival outcomes.

Area of Science:

  • Biochemistry
  • Cell Biology
  • Critical Care Medicine

Background:

  • The role of B-cell lymphoma 2 (Bcl-2), an antiapoptotic protein, in sepsis pathogenesis is not well understood.
  • Limited data exists on blood Bcl-2 concentrations in septic patients and their correlation with outcomes.

Purpose of the Study:

  • To investigate differences in blood Bcl-2 concentrations between survivor and non-survivor septic patients.
  • To determine if blood Bcl-2 levels are associated with sepsis-related mortality.
  • To assess the utility of blood Bcl-2 concentrations in predicting mortality in septic patients.

Main Methods:

  • An observational, prospective study conducted in intensive care units across three Spanish hospitals.
  • Serum Bcl-2 concentrations were measured at the time of sepsis diagnosis.

  • Thirty-day mortality was recorded as the primary outcome variable.
  • Main Results:

    • Non-survivors (n=81) exhibited significantly lower serum Bcl-2 levels compared to survivors (n=140) (p=0.003).
    • Serum Bcl-2 concentrations below 4.4 ng/mL were independently associated with increased mortality (OR=3.228; p=0.006).
    • Blood Bcl-2 levels demonstrated a predictive value for mortality with an area under the curve of 62% (p=0.003).

    Conclusions:

    • This study reveals higher blood Bcl-2 concentrations in sepsis survivors compared to non-survivors.
    • Low blood Bcl-2 concentrations are associated with increased mortality risk in septic patients.
    • Blood Bcl-2 levels show potential as a predictive biomarker for sepsis mortality.
