    Background:

    • Chronic benign granulocytopenia is a rare condition characterized by a severe lack of neutrophils.
    • Neutrophils are critical phagocytes in the innate immune system, primarily responsible for combating bacterial infections.

    Observation:

    • This study investigated the bactericidal activity of monocytes from a child with congenital neutropenia against Escherichia coli and Staphylococcus aureus.
    • Monocyte function was compared to that of monocytes and neutrophils from healthy control subjects.

    Findings:

    • Patient monocytes demonstrated normal bacterial killing capacity comparable to control monocytes against both tested bacteria.
    • At a 1:1 bacteria-to-phagocyte ratio, both patient and control monocytes showed similar efficacy.
    • At a higher ratio (3:1), control neutrophils were significantly more effective than both control and patient monocytes in reducing bacterial load.

    Implications:

    • These findings underscore the crucial functional capacity of monocytes as effective phagocytes, particularly in individuals with neutropenia or neutrophil functional defects.
    • While neutrophils are the primary phagocytes, monocytes play a vital compensatory role in host defense when neutrophil numbers are critically low.
