Summary

Left ventricular aneurysm (VA) does not significantly impact overall cardiac pump function during exercise. However, anterior myocardial infarction (MI) with VA reduces exercise tolerance compared to inferior MI or no VA.

Background:

  • Left ventricular aneurysm (VA) is a potential complication following myocardial infarction (MI).
  • The impact of VA on overall cardiac pump function and exercise capacity requires further elucidation.

Purpose of the Study:

  • To assess the role of left ventricular aneurysm (VA) in affecting cardiac pump function during exercise.
  • To compare exercise tolerance in patients with and without VA after MI.

Main Methods:

  • Maximal symptom-limited exercise testing was performed on 36 patients with echo-proved post-MI VA and 36 matched controls without VA.
  • Patients were analyzed based on MI location (anterior vs. inferior) and ejection fraction (EF).

  • Lower ejection fraction (EF < 30%) was predominantly observed in patients with VA and anterior MI, correlating with lowest exercise tolerance.

Conclusions:

  • Exercise testing is a safe and valuable tool for evaluating the functional capacity of the non-aneurysmatic myocardium in patients with VA.
  • VA, particularly with anterior MI and reduced EF, is associated with diminished exercise tolerance.
