Summary

Minimal deviation adenocarcinoma, a rare and aggressive cervical cancer, is often misdiagnosed. Early and accurate diagnosis is crucial for improving patient survival rates in this challenging malignancy.

Background:

Minimal deviation adenocarcinoma is a rare cervical cancer subtype.
It exhibits high aggressiveness and a propensity for misdiagnosis.
Histopathology is the cornerstone of diagnosis, yet outcomes remain challenging.

Observation:

A 60-year-old perimenopausal woman presented with elevated CA19-9.
Initial examinations were largely unremarkable, with MRI suggesting cervical lesions.
Methylation analysis indicated malignancy, leading to biopsy.

Findings:

Pathologic diagnosis confirmed minimal deviation adenocarcinoma of the cervix.
The tumor was staged as IB2 with lymph node metastasis.
This subtype demonstrates aggressive clinical behavior.

Implications:

Minimal deviation adenocarcinoma has poorer survival than HPV-related cervical adenocarcinoma.
Accurate preoperative diagnosis is vital to prevent treatment delays and improve outcomes.
Individualized treatment approaches are necessary due to the lack of therapeutic consensus.
