Summary

Chronic non-invasive ventilation (NIV) improves daytime hypercapnia in COPD patients and may improve survival in stable COPD. For those post-exacerbation, NIV may prolong admission-free survival but doesn't improve quality of life.

Purpose of the Study:

  • Included randomized controlled trials (RCTs) comparing chronic NIV (≥5 hours/night for ≥3 weeks) with standard care.
  • Analyzed IPD using mixed-effects models and performed meta-analysis on aggregated data.
  • Assessed outcomes at 3 and 12 months, distinguishing between stable COPD and post-exacerbation phases.

Main Results:

  • NIV reduced PaCO2 in both stable and post-exacerbation COPD groups.
  • In stable COPD, NIV showed a probable minor benefit in PaO2 at 3 months and likely reduced all-cause mortality.
  • In post-exacerbation COPD, NIV may prolong admission-free survival but showed little to no benefit on HRQL.

Conclusions:

  • Chronic NIV effectively improves daytime hypercapnia regardless of initiation timing.
  • NIV may improve survival in stable COPD and prolong admission-free survival in post-exacerbation COPD.
  • Further research should identify patient characteristics for treatment success, as RCTs comparing NIV to standard care may no longer be warranted.
