Background:

Basaloid follicular hamartoma (BFH) is a rare benign neoplasm.
BFH requires differentiation from basal cell carcinoma.
Familial multiple BFH (FMBFH) is an inherited subtype presenting in early life.

Observation:

This report details an adult-onset case of FMBFH.
The patient presented with multiple small, skin-colored papules on the central face.
Lesions were cosmetically disfiguring.

Findings:

The adult-onset FMBFH was treated with CO2 laser ablation.
The treatment yielded satisfactory cosmetic and clinical results.
No established standard of care exists for FMBFH treatment.

Implications:

CO2 laser therapy is a viable treatment option for adult FMBFH.
This case expands the understanding of FMBFH presentation and management.
Further research may establish optimal therapeutic strategies for FMBFH.
