Summary

Cervical spinal surgery can lead to C5 nerve root palsy, a rare complication. Combined anterior-posterior surgical approaches in older males with myelopathy showed the highest risk for this condition.

Background:

  • Cervical spinal surgery is generally safe and effective.
  • C5 nerve root palsy is a rare complication, characterized by deltoid and biceps weakness.
  • This palsy is linked to nerve root traction or spinal cord ischemia.

Purpose of the Study:

  • To investigate the clinical and epidemiological features of C5 palsy.
  • To identify if specific surgical approaches are associated with a higher incidence of C5 palsy.

Main Methods:

  • A retrospective analysis of 124 patients undergoing cervical spine surgery over 8 years.
  • Data collected included demographics, diagnosis, surgical approach, and complications.
  • Emphasis was placed on identifying cases of C5 nerve root palsy.

Main Results:

  • Seven patients (5.6%) developed C5 palsy post-surgery.
  • All affected patients were older males with myelopathy.
  • C5 palsy occurred only in patients who underwent combined anterior-posterior or posterior approaches, not anterior-only approaches.

Conclusions:

  • The incidence of C5 nerve root palsy in this cohort was 5.6%.
  • Combined anterior-posterior surgical approaches in older myelopathic males present the highest risk for C5 palsy.
