Background:

  • Synovial sarcoma, typically affecting extremities, is rarely found in the head and neck region.
  • Ethmoidal sinus synovial sarcoma is exceptionally rare, with only three prior reported cases.

Observation:

  • Two patients presented with nasal obstruction, epistaxis, and nasofacial swelling.
  • Endoscopic biopsy and immunohistochemistry confirmed synovial sarcoma in both cases.

Findings:

  • One patient received surgery and chemoradiation; the other, two chemotherapy cycles, succumbing within 6 weeks.
  • Head and neck sarcomas exhibit aggressive behavior and poor prognoses.

Implications:

  • Surgical resection with postoperative radiotherapy is the standard treatment for head and neck sarcomas.
  • High recurrence rates necessitate aggressive management and close patient follow-up for optimal outcomes.
