[Effectiveness and tolerance of an active combination of timolol and pilocarpine--a pilot project]
1Augenklinik und -poliklinik rechts der Isar der Technischen Universität München.
Abstract:
A pilot study was conducted to find out whether it is possible to achieve a further drop in intraocular pressure in eyes with glaucoma, which is about 22 mmHg after treatment with 0.5% timolol, by employing a combination preparation of timolol 0.5% and pilocarpine 2% or timolol 0.5% and pilocarpine 4% respectively, administered as eyedrops. This resulted in an average additional lowering of 1.9 mmHg after 2 hrs and by 2.1 mmHg after 6 hours (TP2) or by 3.0 mmHg after 2 and 3.5 mmHg after 6 and by 2.7 mmHg after 12 hours (TP4), the therapeutically relevant duration of effectivity being 6 (TP2) or 12 (TP4) hours, respectively.
Insights
This pilot study explored adding pilocarpine to timolol eyedrops for glaucoma patients. Combination therapy further reduced intraocular pressure, offering a potential new treatment option.
Area of Science:
- Ophthalmology
- Pharmacology
Background:
- Glaucoma is a leading cause of irreversible blindness.
- Elevated intraocular pressure (IOP) is a primary risk factor for glaucoma progression.
- Timolol 0.5% is a common treatment for lowering IOP.
Purpose of the Study:
- To evaluate the efficacy of combination eyedrops in further reducing IOP in glaucoma patients.
- To compare the effects of timolol with pilocarpine 2% versus timolol with pilocarpine 4%.
Main Methods:
- A pilot study involving glaucoma patients with IOP around 22 mmHg after timolol treatment.
- Administration of combination eyedrops: timolol 0.5% with pilocarpine 2% (TP2) or timolol 0.5% with pilocarpine 4% (TP4).
- Measurement of intraocular pressure at various time points post-administration.
Main Results:
- TP2 resulted in an additional IOP reduction of 1.9 mmHg at 2 hours and 2.1 mmHg at 6 hours.
- TP4 showed a greater additional IOP reduction of 3.0 mmHg at 2 hours, 3.5 mmHg at 6 hours, and 2.7 mmHg at 12 hours.
- Therapeutically relevant IOP lowering duration was 6 hours for TP2 and 12 hours for TP4.
Conclusions:
- Combination therapy with timolol and pilocarpine eyedrops can achieve further IOP reduction in glaucoma patients.
- Timolol 0.5% with pilocarpine 4% demonstrated a more pronounced and sustained IOP-lowering effect compared to timolol 0.5% with pilocarpine 2%.
