Summary

Both interdigital and medial transarticular approaches for lateral release with scarf osteotomy show equally successful outcomes for hallux valgus. The transarticular method offers a single scar benefit, proving safe and effective.

Background:

Purpose of the Study:

  • To compare the clinical and radiographic outcomes of two surgical approaches for lateral release in hallux valgus surgery.
  • To evaluate the efficacy of the interdigital approach versus the medial transarticular approach when combined with scarf osteotomy.

Main Methods:

  • A comparative study involving 77 feet with moderate to severe hallux valgus.
  • Patients underwent scarf osteotomy and lateral soft tissue release via either an interdigital (n=36) or medial transarticular (n=41) approach.
  • Clinical (range of motion, AOFAS score, pain, FAOS) and radiographic (hallux valgus angle, intermetatarsal angle) outcomes were assessed preoperatively and postoperatively (93-124 months).

Main Results:

  • Both groups demonstrated significant improvements in American Orthopaedic Foot & Ankle Society (AOFAS) scores, hallux valgus angle, and intermetatarsal angle.
  • No statistically significant differences were observed between the interdigital and transarticular groups in any clinical or radiographic parameters.
  • Postoperative pain and Foot and Ankle Outcome Score (FAOS) also showed comparable results between the two approaches.

Conclusions:

  • The interdigital and medial transarticular approaches for lateral soft-tissue release combined with scarf osteotomy yield equally successful clinical and radiographic outcomes.
  • The medial transarticular approach is a safe and effective option, offering the advantage of a single scar compared to the interdigital approach.
