Summary

The nasal to temporal amplitudes ratio (N/T) of multifocal electroretinography (mfERG) shows stronger correlations with optical coherence tomography (OCT) and visual field (VF) parameters in glaucoma detection than the photopic negative response to B-wave ratio (PhNR/B). Further research is needed to optimize mfERG for clinical use.

Background:

  • The nasal to temporal amplitudes ratio (N/T) of multifocal electroretinography (mfERG) and the photopic negative response to B-wave ratio (PhNR/B) are potential indicators for detecting glaucomatous changes.
  • While both parameters show promise, their clinical superiority in evaluating glaucoma patients remains undetermined.

Purpose of the Study:

  • To compare the diagnostic efficacy of N/T and PhNR/B parameters derived from mfERG in glaucoma assessment.
  • To evaluate the correlations between mfERG parameters and morphological (OCT) and functional (VF) assessments in glaucoma patients.

Main Methods:

  • Sixty-nine eyes from 44 glaucoma patients underwent morphological assessment using optical coherence tomography (OCT) for ganglion cell complex (GCC) thickness.
  • Functional assessment included Humphrey visual fields (VF) and mfERG measurements for N/T and mfPhNR/B parameters.
  • Correlations between mfERG parameters and OCT or VF measurements were analyzed.

Main Results:

  • The N/T parameter demonstrated significant correlations with GCC thickness and VF parameters, particularly in the inferior and inforotemporal retinal areas.
  • Conversely, the mfPhNR/B parameter showed limited association with OCT and VF parameters, with a significant correlation only found in the inferotemporal sector with GCC thickness.
  • These findings suggest N/T may be more sensitive in detecting glaucomatous changes across multiple retinal areas compared to mfPhNR/B.

Conclusions:

  • The N/T parameter exhibited a broader correlation with GCC and VF measurements than mfPhNR/B in this study.
  • Future research should focus on refining mfERG stimuli and amplitudes to enhance spatial correspondence with OCT and VF for improved clinical evaluation of glaucoma.
