Summary

Rubidium-82 PET/CT effectively visualized pulmonary perfusion in COVID-19 patients with ground-glass opacities. This nuclear imaging technique shows promise for detecting microthrombi and perfusion defects missed by CT angiography.

Background:

  • COVID-19 pandemic presented diagnostic challenges for pulmonary embolism and microthrombi.
  • Chest CT angiography (CTa) can miss subtle pulmonary microthrombi and perfusion abnormalities.
  • Elevated d-dimer levels necessitate further investigation for thromboembolic events.

Observation:

  • A 56-year-old man with shortness of breath showed CO-RADS 5 ground-glass opacities on CTa.
  • Rubidium-82 (82Rb) PET/CT was performed to assess for microthrombi and cardiac perfusion.
  • 82Rb PET/CT revealed pulmonary uptake in ground-glass opacity areas, correlating with CTa findings.

Findings:

  • 82Rb PET/CT demonstrated adequate pulmonary perfusion even in areas of significant ground-glass opacities.
  • No evidence of pulmonary microthrombi was detected by 82Rb PET/CT, despite elevated d-dimer.
  • 82Rb PET/CT findings corresponded well with CTa imaging.

Implications:

  • 82Rb PET/CT is a valuable tool for evaluating pulmonary perfusion in COVID-19.
  • This technique may enhance the diagnostic capabilities beyond conventional imaging for microthrombi.
  • 82Rb PET/CT could improve the detection of minor perfusion defects in the lungs.
