Background:

Chronic intestinal pseudo-obstruction (CIPO) is a rare gastrointestinal motility disorder.
It presents with symptoms mimicking mechanical bowel obstruction but lacks identifiable anatomic lesions.

Observation:

An 86-year-old female presented with severe abdominal discomfort, nausea, and vomiting.
Imaging revealed significant gastric distension without evidence of obstruction.

Findings:

The patient was diagnosed with chronic idiopathic intestinal pseudo-obstruction (CIIP).
This diagnosis was based on clinical presentation and exclusion of other causes.

Implications:

This case underscores the importance of considering CIIP in elderly patients with obstructive symptoms.
Raising awareness may improve diagnostic timelines and patient outcomes for this rare disorder.
