Area of Science:

  • Reproductive Medicine
  • Gynecologic Surgery
  • Maternal-Fetal Medicine

Background:

  • Cesarean scar pregnancy (SEP) is a rare form of ectopic pregnancy occurring at a prior cesarean delivery scar.
  • Untreated SEP can lead to severe maternal morbidity and mortality, including uterine rupture.

Purpose of the Study:

  • To analyze the management and outcomes of 22 cases of cesarean scar ectopic pregnancy (SEP).
  • To evaluate the effectiveness of early diagnosis and treatment strategies for SEP.

Main Methods:

  • Retrospective analysis of 22 SEP cases managed between 2013 and 2020.
  • Diagnosis primarily by transvaginal ultrasonography.
  • Treatment modalities included methotrexate (single or multi-dose) and laparotomy with wedge resection.

Main Results:

  • Mean gestational age at diagnosis was 8.6 weeks.
  • Presentations varied, with 7 asymptomatic cases diagnosed via routine ultrasound.
  • Primary treatment success rate was 91% (20/22 cases), with β-hCG normalization in an average of 53 days.

Conclusions:

  • Early and accurate diagnosis of SEP is crucial for successful management.
  • Conservative treatment options, including methotrexate and surgical scar resection, are effective in preserving fertility and minimizing complications.
  • Prompt intervention significantly reduces maternal morbidity and mortality associated with SEP.
