Background:

Social determinants of health significantly influence health outcomes for clients of local public health agencies.
The Florida Department of Health in Hillsborough County (DOH) initiated a program to address these social needs among its clients.

Purpose of the Study:

To describe a midstream intervention designed to identify and address the social needs of DOH clients.
To evaluate the effectiveness and challenges of connecting clients to community resources.

Main Methods:

The intervention involved screening 768 DOH clients during the 2019 program.
Clients were navigated to community resources to address identified social needs.
Staff and client feedback was collected to identify challenges and facilitators.

Main Results:

77% of screened clients reported having at least one social need.
Clients faced significant challenges navigating resources independently.
The value of having dedicated resource navigators within the agency was recognized.

Conclusions:

Social needs screenings offer a vital opportunity for local public health agencies to enhance client well-being.
The program's findings support the integration of social needs interventions into public health practice.
This approach aligns with community health priorities and promotes health equity.
