Background:

  • Partial nephrectomy (PN) is a standard treatment for renal cell carcinoma (RCC).
  • Minimizing blood loss and operative time is crucial for patient recovery and renal function preservation.
  • Hemostatic agents are often employed to manage bleeding during complex surgical procedures.

Purpose of the Study:

  • To evaluate the efficacy of Hemopatch®, a polyethylene glycol-coated collagen patch, in reducing blood loss and surgical time during PN for RCC.
  • To assess the impact of Hemopatch® on renal function post-PN.
  • To compare outcomes between conventional PN (CPN) and PN using Hemopatch® (HPN).

Main Methods:

  • A cohort of 42 patients undergoing PN with Hemopatch® (HPN) was compared to 565 patients undergoing conventional PN (CPN).
  • Data collected included patient demographics, tumor characteristics, blood loss, surgical duration, and renal function (CKD-EPI).
  • Statistical analysis was performed to compare outcomes between the HPN and CPN groups.

Main Results:

  • The HPN group demonstrated significantly lower median blood loss (114 ml ± 159 ml) compared to the CPN group (146 ml ± 195 ml) (p=0.021).
  • Surgical duration was also significantly shorter in the HPN group (43 min ± 27 min) versus the CPN group (53 min ± 49 min) (p=0.035).
  • No significant difference was observed in renal clamping time or a significant decline in renal function post-surgery.

Conclusions:

  • Hemopatch®-supported renoraphy shows promising results in partial nephrectomy, leading to reduced blood loss and surgical time.
  • No adverse side effects were reported with the use of Hemopatch®.
  • Hemopatch® should be considered for complex partial nephrectomies, with further studies recommended to investigate its role in preventing fistulas.
