Summary

Inflammation in first-episode psychosis (FEP) was studied using cerebrospinal fluid (CSF) and blood tests. While some alterations were found, no significant link between peripheral inflammation and CSF inflammation was established in this large FEP cohort.

Purpose of the Study:

  • To investigate the presence and relationship of inflammatory biomarkers in cerebrospinal fluid (CSF) and blood of first-episode psychosis (FEP) patients.
  • To analyze the largest available FEP cohort for routine CSF and blood diagnostics related to inflammation.

Main Methods:

  • Retrospective analysis of clinical data from FEP inpatients diagnosed with schizophrenia-spectrum disorders (ICD-10 F2x).
  • Inclusion criteria: patients admitted between January 1, 2008, and August 1, 2018, who underwent lumbar puncture.
  • Analysis included CSF alterations (oligoclonal bands, albumin ratios) and serum C-reactive protein (CRP) levels.

Main Results:

  • A total of 314 FEP patients were analyzed; 42.7% showed CSF alterations.
  • Cerebrospinal fluid (CSF) findings included oligoclonal bands (OCBs) in 21.8% and blood-brain-barrier (BBB) dysfunction in 15.8%.
  • Elevated serum CRP levels were observed in 41.4% of patients, with a mean level of 2.4 mg/l.

Conclusions:

  • This large FEP cohort analysis provides extensive data on inflammation markers in psychosis.
  • Despite detecting inflammatory alterations in both CSF and blood, no significant relationship was found between peripheral inflammation and CSF inflammation.
  • No significant association was established between CSF alterations and peripheral inflammation as measured by CRP.
