  • Initial imaging suggested a cavernous sinus hemangioma, but re-evaluation revealed an internal carotid artery outpouching.
  • Leukocytosis and elevated inflammatory markers indicated an infectious or inflammatory process.

Findings:

  • The patient was diagnosed with a mycotic aneurysm of the intracavernous internal carotid artery.
  • This diagnosis was based on a combination of clinical presentation and advanced neuroimaging findings.

Implications:

  • Mycotic aneurysms are rare but life-threatening vascular infections requiring high clinical suspicion.
  • Prompt diagnosis and management are essential to prevent catastrophic complications such as rupture or stroke.
