Low-grade fibromyxoid sarcoma (LGFMS) is a rare, aggressive cancer. This case highlights LGFMS in the maxillofacial region, emphasizing the need for vigilance and long-term monitoring for this indolent tumor.

Background:
- Low-grade fibromyxoid sarcoma (LGFMS) is a rare soft tissue neoplasm with deceptively benign histologic features but a propensity for metastasis.
- While typically affecting the deep soft tissues of the trunk and extremities in adults, LGFMS rarely occurs in the maxillofacial region.
Observation:
- A case of LGFMS in the left lower face of a 35-year-old male is presented, with a 6-month history.
- Gross examination revealed a 2 cm × 2 cm × 1 cm ovoid mass. Histopathology showed a well-circumscribed myxoid tumor with areas of hypocellularity and collagenization.
Findings:
- Immunohistochemical analysis demonstrated diffuse vimentin positivity and negative staining for desmin and S-100 protein, confirming the LGFMS diagnosis.
- The patient underwent surgical excision followed by 30 cycles of radiotherapy. No recurrence or metastasis was observed at 1-year follow-up.
Implications:
- This case underscores the importance of recognizing LGFMS in unusual locations like the maxillofacial region.
- The indolent yet metastatic nature of LGFMS necessitates long-term clinical follow-up to monitor for potential recurrence or distant spread.
