Summary

For men with non-obstructive azoospermia (NOA), microdissection testicular sperm extraction (micro-TESE) offers the most effective and safest method for sperm retrieval. This advanced technique provides hope for successful conception where previously options were limited.

Background:

Non-obstructive azoospermia (NOA) affects 10-15% of infertile men, historically limiting parenthood options to donor sperm or adoption.
Recent advancements in sperm extraction and assisted reproductive technologies offer new possibilities for NOA patients.

Purpose of the Study:

To review current principles for examining and preparing NOA patients for surgical sperm retrieval.
To compare the efficacy and safety of various testicular biopsy techniques.

Main Methods:

A comprehensive review of 52 articles from the MEDLINE database (PubMed).
Analysis focused on diagnostic testicular biopsy and comparative data on sperm retrieval methods.

Main Results:

Microdissection testicular sperm extraction (micro-TESE) is identified as the most effective and safest sperm retrieval technique currently available.
While micro-TESE can be complex, it achieves successful outcomes in challenging cases of male infertility.

Conclusions:

Micro-TESE represents a significant advancement in treating male infertility due to non-obstructive azoospermia.
This technique expands treatment possibilities, offering a chance for biological parenthood to previously infertile men.
