Summary

Endovascular repair of aortic dissection is challenging due to flap defects. This study presents a minimally invasive technique to successfully reroute mispositioned endoprostheses into the true lumen, improving patient outcomes.

Background:

  • Anatomic complexity of aortic dissection presents challenges for endovascular repair.
  • Defects in the dissection flap can lead to guidewire misplacement between lumens.
  • Inadvertent placement of endoprostheses into the false lumen is a known complication.

Observation:

  • Accidental guidewire passage and device malpositioning into the false lumen occurred in patients undergoing endovascular aortic repair.
  • Existing literature offers limited descriptions of this complication and its management.
  • Seven patients experienced errant endoprosthesis placement into the false lumen.

Findings:

  • A novel minimally invasive technique was employed to address endoprosthesis malpositioning.
  • The technique successfully rerouted the mispositioned endoprosthesis into the true lumen in all 7 patients.
  • Successful repositioning was achieved with minimal invasiveness.

Implications:

  • This approach offers a viable bailout strategy for endoprosthesis malpositioning in aortic dissection.
  • Minimally invasive techniques can effectively manage complications in complex aortic repairs.
  • Further research may validate this technique for broader application in endovascular aortic surgery.
