Summary

Robot-assisted radical prostatectomy (RARP) offers comparable outcomes to open surgery. The extraperitoneal approach, especially with single-port systems, may provide benefits like faster recovery and improved cosmesis.

Area of Science:

  • Urology
  • Minimally Invasive Surgery
  • Robotic Surgery

Background:

  • Robot-assisted radical prostatectomy (RARP) is the standard minimally invasive treatment for localized prostate cancer.
  • The transperitoneal approach is common, but the extraperitoneal approach offers benefits in specific cases.
  • Single-port (SP) robotic systems are increasingly used for extraperitoneal RARP.

Purpose of the Study:

  • To describe the technical steps of extraperitoneal single-port RARP.
  • To review the clinical outcomes associated with this approach.

Main Methods:

  • Description of the technical procedure for extraperitoneal single-port RARP.
  • Literature review of reported clinical outcomes.

Main Results:

  • Extraperitoneal RARP, particularly with SP systems, may offer advantages such as better cosmesis and reduced opioid use.
  • Comparable oncologic outcomes have been reported for both transperitoneal and extraperitoneal approaches.
  • Potential benefits include minimal bowel manipulation and faster bowel function recovery.

Conclusions:

  • Extraperitoneal single-port RARP is a viable alternative, especially when intraperitoneal access is challenging.
  • This approach may lead to enhanced patient recovery and cosmesis.
  • Further research into its widespread adoption and long-term outcomes is warranted.
