Background:

Supernumerary lacrimal punctum is a rare congenital anomaly.
While often asymptomatic, it can lead to symptoms like excessive tearing or canaliculitis.

Observation:

Four distinct cases of supernumerary lacrimal puncta are presented.
Patients presented for various reasons including eye lesions, diabetic retinopathy, cataract follow-up, and retinal clinic visits.
The accessory puncta were located on the lower eyelid and, in one instance, shared a canaliculus.

Findings:

Probing revealed an immediate horizontal course of the accessory canaliculus, differing from the typical vertical-then-horizontal path.
A soft stop was noted approximately 4 mm into the accessory punctum during probing.

Implications:

Ophthalmologists must be aware of this rare anatomical variation.
Early recognition can prevent misdiagnosis and guide appropriate management of associated symptoms.
Further research may elucidate the embryological basis and clinical significance of this anomaly.
