Peripheral visual field testing is crucial for glaucoma diagnosis, as central tests alone miss defects in 18% of cases. Combining central and peripheral visual field tests offers a more comprehensive glaucoma assessment.

Background:

  • Glaucoma visual field defects exhibit significant inter-individual variability.
  • Central visual field testing alone may miss critical diagnostic information in glaucoma patients.

Purpose of the Study:

  • To compare central and peripheral visual field defects in glaucoma suspects and primary open-angle glaucoma (POAG) patients.
  • To determine if central visual field testing alone results in the loss of clinically valuable information.

Main Methods:

  • Prospective observational study of 167 eyes from 89 patients with glaucoma or suspect glaucoma.
  • Static automated perimetry (SAP) followed by peripheral kinetic visual field testing using Octopus 900 (Goldmann III4e stimuli).

Main Results:

  • 18% of eyes with normal central visual fields showed glaucomatous peripheral defects.
  • 86% of glaucoma patients with moderate-to-severe central defects had corresponding peripheral defects.
  • Moderate correlation found between central and peripheral visual fields; peripheral testing was significantly faster (71s vs 803s).

Conclusions:

  • Glaucomatous visual field defects are diverse, and relying solely on central testing risks missing vital clinical information.
  • Peripheral kinetic perimetry is a feasible adjunct to central static perimetry for comprehensive glaucoma visual function assessment.
