Summary

Long-acting muscarinic antagonists (LAMA) monotherapy in asthma patients is common and linked to a higher risk of severe asthma exacerbations (SAE). LAMA should always be prescribed with inhaled corticosteroids (ICS) to ensure patient safety.

Background:

Purpose of the Study:

  • To determine the prevalence of LAMA monotherapy in asthma patients.
  • To investigate the association between LAMA monotherapy and severe asthma exacerbations (SAE).

Main Methods:

  • A cohort study utilizing the IPCI primary care database (2007-2017) included asthma patients aged 6-50.
  • Treatment periods were categorized as LAMA monotherapy, dual therapy (LAMA + ICS), or triple therapy (LAMA + ICS + LABA).
  • Relative rates (RR) of SAE were estimated, adjusting for patient characteristics.

Main Results:

  • Of 66,508 asthma patients, 1.9% used LAMA. LAMA monotherapy accounted for 38.7% of LAMA treatment periods.
  • LAMA monotherapy showed a RR of 1.5 for SAE compared to dual therapy.
  • Patients alternating between LAMA monotherapy and dual therapy (without triple therapy) had a significantly increased RR of 5.7 for SAE.

Conclusions:

  • LAMA is frequently prescribed as monotherapy without concurrent inhaled corticosteroids (ICS).
  • LAMA monotherapy is associated with an increased risk of asthma exacerbations.
  • LAMA should not be prescribed without concomitant ICS use in asthma patients.
