    Background:

    • Acute myelomonocytic leukemia (AMML) can relapse in the central nervous system (CNS).
    • High-dose cytarabine (Ara-C) is used in treating hematological malignancies, including AMML.
    • CNS leukemia relapse often requires intensive therapeutic strategies.

    Observation:

    • Two patients with CNS relapse of AMML developed leukoencephalopathy after high-dose intravenous Ara-C.
    • Clinical and neuroimaging findings consistent with leukoencephalopathy appeared 5-7 days post-therapy.
    • One patient received combined therapy (intravenous Ara-C, cranial irradiation, intrathecal Ara-C); the other received prior intrathecal chemotherapy.

    Findings:

    • Leukoencephalopathy in the first patient was likely due to synergistic effects of triple therapy.
    • Intravenous Ara-C was the probable precipitating factor in the second patient.
    • Altered mental status and white matter changes on CT scan indicated leukoencephalopathy.

    Implications:

    • High-dose Ara-C, particularly in combination regimens, poses a risk of neurotoxicity.
    • Understanding Ara-C induced leukoencephalopathy mechanisms is crucial for patient management.
    • This highlights the need for careful monitoring of neurological function during intensive chemotherapy for CNS leukemia.
