Continuous ambulatory peritoneal dialysis and acute appendicitis.

    Summary

Peritonitis in continuous ambulatory peritoneal dialysis (CAPD) patients can stem from skin contamination or intra-abdominal issues. Acute appendicitis is a critical, often overlooked, cause requiring surgical intervention.

    Area of Science:

    • Nephrology
    • Infectious Diseases
    • Surgical Pathology

    Background:

    • Continuous ambulatory peritoneal dialysis (CAPD) is a common treatment for end-stage renal disease.
    • Peritonitis is a serious complication of CAPD, typically caused by skin organism contamination of the dialysis line.
    • However, intra-abdominal pathology can also precipitate peritonitis in CAPD patients.

    Observation:

    • This study presents three cases of CAPD-associated peritonitis.
    • In these cases, the peritonitis was specifically caused by acute appendicitis.
    • The clinical presentation and diagnostic challenges are highlighted.

    Findings:

    • Acute appendicitis can present as peritonitis in CAPD patients, mimicking typical dialysis-related infections.
    • Prompt recognition of an intra-abdominal source is crucial for effective management.

    Related Experiment Videos

  • Surgical intervention is often necessary for definitive treatment.
  • Implications:

    • Clinicians managing CAPD patients must consider intra-abdominal pathology, such as appendicitis, in cases of peritonitis.
    • Early surgical consultation and intervention can improve outcomes for CAPD patients with non-dialysis line related peritonitis.
    • This underscores the importance of a comprehensive diagnostic approach in CAPD-related peritonitis.
