Chronopharmacokinetics: Circadian Rhythms and Influence on Drug Response

160
Circadian rhythms are cyclic changes that are crucial in plasma drug concentrations. Various standard circadian parameters, including core body temperature, heart rate, and other cardiovascular factors, directly impact disease states and the therapeutic response to drug therapy.
The time of drug administration is an important factor to consider, as it can influence the toxic dose of a drug. For example, a study conducted by Prins et al. in 1997 examined the effects of the timing of...

Vascular Resistance01:20

Vascular Resistance

7.8K
Vascular resistance is a critical concept in understanding blood flow dynamics in the circulatory system. It refers to the resistance that blood encounters as it flows through the blood vessels. This resistance is a key factor in determining blood pressure and cardiac workload.
The primary determinants of vascular resistance are vessel diameter, blood viscosity, and vessel length. Among these, vessel diameter plays the most significant role due to the fourth power relationship described by...

Summary
This summary is machine-generated.

Vascular endothelial function, measured by reactive hyperemia index (RHI), was not linked to fatigue or sleep duration in healthy adults. Further research is needed to understand RHI in severe fatigue states.

Area of Science:

  • Cardiovascular physiology
  • Endothelial function assessment
  • Occupational health

Background:

  • The reactive hyperemia index (RHI) via peripheral arterial tonometry (PAT) correlates with flow-mediated dilation.
  • Low RHI is a mortality risk factor in high-risk cardiovascular disease (CVD) patients.
  • CV events are linked to stress, fatigue, and sleep deprivation, but their effect on endothelial function is unclear.

Purpose of the Study:

  • To investigate the relationship between fatigue, sleep duration, and endothelial function in healthy adults.
  • To determine if overnight duty impacts vascular endothelial function.

Main Methods:

  • 13 healthy hospital workers (31.6 years avg. age) underwent 72 reactive hyperemia-peripheral arterial tonometry (RH-PAT) measurements.
  • Sleep hours and fatigue (visual analog scale - VAS) were recorded before/after duty.
  • RHI was measured using RH-PAT.

Main Results:

  • RHI did not differ significantly between measurements taken before and after duty, despite differences in sleep and fatigue.
  • Higher VAS scores were observed in participants with low RHI (<1.67) compared to normal RHI (≥2.07).
  • Adjusted regression analyses showed no significant association between low RHI and VAS scores or sleep duration.

Conclusions:

  • Vascular endothelial function, assessed by RHI, was not associated with overnight duty, sleep duration, or fatigue in healthy young adults.
  • Autonomic nerve activity might influence RHI in severe fatigue; consider examinee's condition in RH-PAT interpretation.
